The Following forms are REQUIRED to fulfill registration requirements:
Participants
1. PROOF of age - Copy of Certified birth certificate
2. 2009 Panther Registration Form
3. 2009 Pop Warner Participant Contract
4. 2009 Medical/Physical Form (**must be dated after January 1, 2009!)
5. 2008-2009 COMPLETE report card. (with promotion status included)
NO Participants will be allowed on the practice field without ALL required paperwork. |
